2013 HealthPartners “Partners in Excellence” Awards

Once again, Entira Family Clinics was recognized for achieving high levels of performance on the Triple Aim (clinical quality, patient experience and affordability) at the recent HealthPartners annual “Partners in Excellence” Awards dinner!  These awards are based on performance measurements provided by Minnesota Community Measurement (MNCM) and HealthPartner’s claims.  The honors categories where we excelled are listed below:

Gold Honors:
      “Attributed Total Cost of Care1”  
      “Drug Monitoring of Persistent Medications; Primary Care2.”  
Silver Honors:
       “MNCM Optimal Depression Care; Primary Care3
       “Patient, Primary Care: Informed about Your Needs”
       “Patient Experience, Primary Care: Willingness to Recommend.” 

Following the 2012 awards dinner, additional measurements were released by MNCM. Entira Family Clinics received the Silver honor in “MNCM Controlling High Blood Pressure4.”

1Attributed Total Cost of Care is a measure of efficiency, intensity and the price of care delivered compared to the average for similar care providers. “Total cost” includes all care such as lab tests, x-rays and care from specialists and hospitals.
2Drug Monitoring of Persistent Medications includes members 18 years and older on Angiotensin Converting Enzyme (ACE) inhibitors, or Angiontensin Receptor Blockers (ARB) and/or diuretics who received at least one appropriate monitoring test in the measurement year.
3MNCM Optimal Depression Care includes the percentage of members age 18 and older with a diagnosis of major depression in 2013 that were treated with antidepressant medication and met all three goals: documentation of five or more DSM-IV defined symptoms, documented monitoring of three or more symptoms in follow-up visits within three months of diagnosis, and maintained on antidepressant medication for six months.
4 MNCM Controlling High Blood Pressure includes members 18-85 who had a diagnosis of hypertension and whose blood pressure was adequately controlled to less than 140/90 during the measurement year.

Congratulations to Entira Family Clinics for its “Partners in Excellence” Awards!

Once again, Entira Family Clinics achieved high levels of performance on the Triple Aim (clinical quality, patient experience and affordability) from HealthPartners annual “Partners in Excellence” Awards.  We received the Gold honor for our “Attributed Total Cost of Care*” and the Silver honor for “MNCM Optimal Vascular Care**”, “Patient, Primary Care: Informed about Your Needs” and “Patient Experience, Primary Care: Willingness to Recommend.”

**MNCM Optimal Vascular Care includes members ages 18 to 75 with vascular disease who met all four goals: Blood pressure less than 130/80 or less than 140/90 for members with diabetes lower than 100mg/dl, non-smoker and daily aspirin use.
